Being listed as an accepted insurance carrier does not guarantee that every plan offered by that carrier is in network. Patients are responsible for confirming coverage, referrals, copayments, deductibles, and other benefits directly with their insurance company.

Self-Pay Options
We welcome patients without insurance and patients who prefer to pay directly for their care. Self-pay options are available for many services, including new patient visits, same-day sick visits, follow-up visits, physicals, telemedicine, medical forms and clearances, weight management, and select wellness services.

Payment Information
Copayments and other patient-responsibility amounts are due at the time of service. We accept major credit cards, debit cards, and eligible HSA or FSA cards.
